RESUMO

Conocer la historia, como en cualquier ámbito de la vida, es necesario para saber no sólo de dónde venimos sino comprender el porqué de la actualidad. También nos sirve de guía, como proyección hacia un futuro mucho mejor para nuestra profesión y, por ende, para las personas que podamos encontrar en el camino. Gracias a dicha historia, desde el COPTOA planteamos una necesidad de hablar de la terapia ocupacional a otros profesionales, a posibles beneficiarios, a los gestores de salud, educación o servicios sociales. O como dice el lema de este año de la Federación Mundial de Terapeutas Ocupacionales “Unidad a través de la comunidad”.(AU)


Knowing history, as in any area of life, is necessary to know not only where we come from but to understand why we are today. It also serves as a guide for us, as a projection towards a much better future for our profession and, therefore, for the people we may meet along the way. Thanks to this history, at COPTOA we raise the need to talk about occupational therapy to other professionals, to possible beneficiaries, to health, education or social services managers. Or as this year's motto of the World Federation of Occupational Therapists says, “Unity through Community”.(AU)

Occupational therapy knowledge emerged in the 19th century as reformist movements responded to the industrialisation of society and capitalist expansion. In the Global North, it was institutionalised by State apparatuses during the First and Second World Wars. Although biomedicine contributed to the rapid expansion and establishment of occupational therapy as a health discipline, its domestication by the biomedical model led to an overly regulated profession that betrays its reformist ideals. Drawing on the work of Deleuze and Guattari, our aim in this article is to deconstruct the biomedicalisation of occupational therapy and demonstrate how resistance to this process is critical for the future of this discipline. The use of arts and crafts in occupational therapy may be conceptualised as a 'nomad science' aesthetically resisting the domination of industrialism and medical reductionism. Through the war efforts, a coalition of progressive nurses, social workers, teachers, artisans and activists metamorphosed into occupational therapists. As it did with nursing, biomedicine proceeded to domesticate occupational therapy through a form of 'imperial' patronage subsequently embodied in the evidence-based movement. 'Occupational' jargon is widely used today and may be viewed as the product of a profession trying to establish itself as an autonomous discipline that imposes its own regime of truth. Given the symbolic violence underlying this patronage, the future of occupational therapy should not mean behaving according to biomedicine's terms. As a discipline, occupational therapy must resist the appropriation of its 'war machine' and craft its own terms through the release of new creative energy.

La AOTA ha publicado la Cuarta Edición del Marco de Trabajo para la Práctica de la terapia ocupacional: Dominio y Proceso. En este artículo vamos a hacer un recorrido por su historia desde el primer Marco de Trabajo publicado en el año 2002 pasando por cada una de sus revisiones realizadas cada 5 años hasta llegar a este cuarto Marco de Trabajo publicado el pasado año 2020. El objetivo es poder conocer tanto sus inicios como la evolución del mismo para lo que nos remontaremos a sus antecedentes: la Terminología Uniforme. (AU)


The objective is to be able to know both its beginnings and its evolution for which we will go back to its antecedents: the Uniform Terminology. The AOTA has published the Fourth Edition of the Framework for the Practice of Occupational Therapy: Domain and Process. In this article we are going to go through its history from the first Framework published in 2002, focusing on each review carried out every 5 years, ending with this fourth Framework published in 2020. The objective is to know both its beginnings and its evolution, and we go back to its antecedents: the Uniform Terminology. (AU)

In 2020 the new Occupational Therapy Practice Framework: Domain and Process, 4th edition was published after reviewing the bases and processes of occupational therapy. En un momento en el que las profesiones sanitarias evolucionan prestando cada vez más importancia a la función y participación ocupacional, la AOTA ha publicado la última actualización de su Marco de Trabajo. El objetivo de este artículo es crear una discusión sobre cómo esta actualización se relaciona con otras teorías, marcos o modelos utilizados actualmente en terapia ocupacional. A pesar de las características únicas de cada teoría, el Marco de la AOTA continúa cercano a los modelos más tradicionales de la profesión en cuanto a valores, conceptos, evolución y enfoque. Sin embargo, se ha desarrollado menos que otrosmodelos actuales en cuanto aevidencia científica y el uso de varios enfoques o conocimiento externo a la profesión. (AU)


As health care professions move towards emphasizing function and occupational participation, AOTA has published the last version of its Occupational Therapy Framework. The aim of this article is to facilitate a discussion about how the framework relates toother actual theories, frameworks or models in occupational therapy. Despite the unique characteristics of each theory, the framework continues in line with the profession traditional models in relation to values, concepts, development and focus. However,the framework lacks evidence-based practice and the use of a variety of approaches or related knowledge to the profession than actual models. (AU)

La revisión de los diferentes Marcos de Trabajo para la Práctica de la Terapia Ocupacional: dominio y proceso (OTPF) de la Asociación Americana de Terapia Ocupacional (AOTA) nos proporciona, ya desde su primera edición, continuas referencias a la Clasificación Internacional del Funcionamiento, Discapacidad y Salud (CIF).A través de este artículo pretendemos analizar la relación que existe entre el Marco de Trabajo para la Práctica de la Terapia Ocupacional de la AOTA y la Clasificación Internacional del Funcionamiento, Discapacidad y salud (CIF) y responder a una serie de cuestiones relativas a esta relación, llevando a cabo una revisión de artículos que versen sobre estos temas en las principales bases de datos de ciencias de la salud. The review ofthe different Frameworks for the Practice of Occupational Therapy: domain and process (OTPF) of the American Occupational Therapy Association (AOTA) provides us, since its first edition, continuous references to the International Classification of Functioning, Disability and Health (ICF). In this article we intend to analyze the relation that exists between the AOTA Framework for the Practice of Occupational Therapy and the International Classification of Functioning, Disability and Health (ICF) and answersome questions related to this relationship. La promoción de la salud se considera como uno de los objetivos principales de la práctica de Terapia Ocupacional. Las similitudes entre promoción de la salud y nuestra disciplina han sido ampliamente reconocidos y discutidos desde diferentes fuentes institucionales y empíricas, pero su implementación puede resultar compleja debido a múltiples factores, incluyendo una confusión en los conceptos y estrategias de acción de la misma. El objetivo de este trabajo es proporcionar un contexto sobre la promoción de la salud y su relación con la Terapia Ocupacional, abordando reflexivamente diferentes aspectos relacionados con la capacidad potencial de la ocupación como promotora de salud. Finalmente, se describen cómo las modificaciones incorporadas a la nueva edición del Marco de Trabajo para la Práctica de Terapia Ocupacional publicado por la Asociación Americana de Terapia Ocupacional pueden contribuir a consolidar las oportunidades de intervención de los terapeutas ocupacionales en el marco de la promoción de la salud individual y pública. Health promotion is considered a main purpose of Occupational Therapy practice. Similarities between health promotion and our discipline have been widely recognized and discussed through different institutional and empirical sources, but implementation of health promotion can be difficult due to multiple factors. Confusion regarding conceptual aspects or action strategies can constitute a barrier to health promotion in Occupational Therapy. The purpose of this work is to provide context about health promotion and its relationship with Occupational Therapy, by critically addressing different aspects about the health-promotion potential value of occupations. Finally, we describe how newly introduced modifications in the Fourth Edition of the Occupational Therapy Practice Framework of the American Occupational Therapy Association can contribute to consolidate intervention opportunities for occupational therapy in individual and public health promotion. Es un documento en constante evolución, que fue creado originariamente para describir la singular perspectiva y la contribución de terapia ocupacional en la promoción de la salud y el compromiso de las personas en la participación en ocupaciones. Pretende ser una herramienta para la comunicación con la población y otros profesionales, proporcionando un lenguaje que lo permita. El objetivo de éste artículo es reflexionar y analizar si el marco cumple el objetivo de proporcionar un lenguaje uniforme y conocer otras propuestas de lenguaje profesional. (AU)


The Occupational Therapy Framework of the AOTA reaches to its fourth edition. This is an ever-evolving document that was originally developed to describe occupational therapy ́s distinct perspective and contribution to promoting health and engagement of persons participating in occupations. It intends to be a tool for the communication with population and others professionals, providing a language to facilitate it. The aim of this article is to reflect and analyze whether the framework fulfills the objective of providing a uniform language and to learn about other professional languages proposals. (AU)

En 2020 la Asociación Americana de Terapia Ocupacional (AOTA) publicó la cuarta edición de su Marco de Trabajo para la Práctica de Terapia Ocupacional: Dominio y Proceso. Más de 40 años hace ya del lanzamiento de su predecesor, la Terminología Uniforme de Terapia Ocupacional, creado con una finalidad diferente a la actual. Desde entonces y hasta ahora, es evidente la evolución de este documento a lo largo de sus diferentes ediciones para tratar de adaptarse a los cambios y necesidades de la profesión y reflejar la evolución en la práctica, siendo objeto tanto de alabanzas como de críticas. El presente artículo pretende analizarlas diferentes críticas recogidas en la literatura de terapia ocupacional en relación a las diversas ediciones del Marco de Trabajo para la Práctica Profesional de la AOTA y sus documentos predecesores (Terminología Uniforme de Terapia Ocupacional I, II y III). Se recoge también una crítica somera de las autoras de este artículo y se invita a abrir un espacio para la reflexión y futuras investigaciones acerca del uso y validez de este documento para la globalidad de la comunidad de terapia ocupacional. In 2020, American Occupational Therapy Association (AOTA) published the fourth edition of its Occupational Therapy Practice Framework: Domain and Process. Its predecessor, The Uniform Occupational Therapy Terminology, was created and launched more than 40 years ago with a different purpose than the current one. Since then until now, the document development is clear through its different editions while trying to adapt to changes and needs of the profession and to reflect the evolution in practice, being the object of both praise and criticism. This article tries to analyze different criticisms gathered in occupational therapy literature regarding the different editions of AOTA’s Occupational TherapyPractice Framework and its previous documents (Uniform Occupational Therapy Terminology I, II and III). It also includes a brief critique by the authors of this article and invites to an open space for reflection and future research on the use and validity of this document for the occupational therapy community as a whole. La evaluación ocupacional, supone el principio de cualquier intervención en terapia ocupacional. Coincidiendo con las ideas de Rogers y Holm, el terapeuta ocupacional que va a iniciar su intervención, no comienza de cero. Los conocimientos teóricos y habilidades prácticas que cada profesional pone en juego durante la evaluación y la intervención, son resultado de su formación y desarrollo paulatino, resultado de la interacción entre su experiencia y su formación teórica. En este documento podremos ver de forma específica como elaborar una evaluación ocupacional utilizando los componentes desarrollados en el marco de trabajo para la práctica de la terapia ocupacional. Occupational evaluation is the beginning of any intervention in occupational therapy. Coinciding with the ideas of Rogers and Holm, the occupational therapist who is going to start his intervention, does not start from scratch. The theoretical knowledge and practical skills that each professional puts into play during the evaluation and intervention are the result of their training and gradual development, the result of the interaction between their experience and their theoretical training. In this document we can see specifically how to develop an occupational evaluation using the components developed in the framework for the practice of occupational therapy. (AU)

When occupational therapists generate hypotheses, we are outlining our lines of reflection in favor of professional practice, and we are also clearly defining: what, how, where and when. This hypothesis management, in the form of diagnostic reasoning, generates relevant answers to determine the importance of our professional language. Su diseño se articula alrededor de la selección de diversos abordajes que estructuran las técnicas, medios, modalidades y métodos que nos permiten realizar la intervención para cada caso abordado. OBJETIVOS: construir el perfil biográfico de una de las mujeres fundadoras y principales representantes de la terapia ocupacional en Chile: Brígida Flores. MÉTODO: desde un paradigma interpretativo, por medio del enfoque biográfico e incorporando elementos de la teoría fundamentada, se realiza el rescate de la historia oral por medio de entrevistas semiestructuradas en profundidad a personas cercanas que tuvieron relación con Brígida Flores. Además, se analizan textos académicos publicados y no publicados, material audiovisual, entre otros documentos. RESULTADOS: se realiza una codificación de la información por medio de intervalos cronológicos, generando la construcción de un perfil biográfico en base a la saturación de la información. Junto con ello, se incorporan elementos contextuales de la época en que Brígida Flores ejerció como terapeuta ocupacional y académica. Así, se identifican diferentes momentos de su historia. CONCLUSIONES: se destaca a Brígida Flores como una de las mujeres más relevantes e influyentes en los inicios de la profesión. Su historia ha trascendido diferentes generaciones y lugares del país. Sus enseñanzas son parte de la identidad profesional de muchas/os terapeutas ocupacionales de Chile


OBJECTIVE: To write the biographical profile of one of the main female founders and most representative figures in Occupational Therapy in Chile: Brígida Flores. METHOD: From an interpretive paradigm, through the biographical approach and incorporating elements of grounded theory, the rescue of oral history is carried out through in-depth semi-structured interviews with close people who had a relationship with Brígida Flores. Published and unpublished academic texts, audiovisual material, among other documents are also analysed. RESULTS: A codification of the information is carried out by means of chronological intervals, generating the construction of a biographical profile based on the saturation of the information. Along with it, contextual elements from the time when Brígida Flores was an occupational therapist and academic are incorporated. Thus, different moments in its history are identified. CONCLUSIONS: Brígida Flores stands out as one of the most relevant and influential women in the early stages of occupational therapy in Chile. Her history has transcended different generations and places in the country. Her teachings are part of the professional identity of many occupational therapists in Chile

The profession of occupational therapy was formalized in the USA in 1917. Many of its earliest proponents were psychiatrists, yet their role in the development of the profession has received limited attention. This paper addresses this gap by considering one of the earliest Irish psychiatrist patrons of occupational therapy: Dr Eamon O'Sullivan (1897-1966) of Killarney Mental Hospital, Co Kerry, who developed an occupational therapy department in 1934. A textbook written by O'Sullivan reflects core philosophies articulated by occupational therapy's founders, and these philosophies were evident in practice at his hospital. Some inconsistencies between O'Sullivan's writings and practice are identified. In the absence of patient testimonies, it is not possible to resolve questions about the potential exploitation of patients through work as therapy.

Health professions education is increasingly creating learning experiences after one's entry-level practice degree. Such experiences include residency and fellowship experiences for health professions practitioners. This review of residency and fellowship programs across several health professions includes the development of residency programs and the implications these developments have on occupational therapy. The analysis across health professions includes medicine, pharmacy, physical therapy, speech-language pathology, and occupational therapy. This thorough analysis can help guide the development of occupational therapy fellowships. This background provides a foundation to focus on the implications for emerging fellowships within occupational therapy. The findings can be utilized to assist in the development of new successful fellowship programs.

Neste artigo, realiza-se um estudo historiográfico sobre o psiquiatra Heitor Péres, quem, dentre outras funções assumidas no serviço público, dirigiu a Colônia Juliano Moreira entre 1946 e 1956. A pesquisa teve como interesse principal investigar suas contribuições na área da praxiterapia, isto é, o uso de ocupações terapêuticas no âmbito da assistência psiquiátrica. Heitor Péres foi um grande entusiasta do que denominou praxiterapia integral. Sua trajetória profissional auxilia a compreender o lugar desta prática no campo da medicina psiquiátrica no Rio de Janeiro. As conclusões deste estudo sugerem o emprego paralelo de teorias organicistas e métodos de tratamentos com base nas ocupações terapêuticas em sua prática profissional. A trajetória de Heitor Péres ajuda a problematizar a leitura histórica que se tem feito da psiquiatria brasileira das décadas de 1940 e 1950, assim como a desmistificar a ideia de que a praxiterapia era percebida neste contexto como um método de tratamento inferior àqueles de base organicista. In this article, a historiographical study is performed about the psychiatrist Heitor Péres, who, among other functions assumed in the public service, directed the Colonia Juliano Moreira between 1946 and 1956. The research had as main interest to investigate the contributions of this character to praxitherapy, or the use of therapeutic occupations within psychiatric care. Heitor Péres was a great promoter and disseminator of what he called integral praxitherapy, in addition to art therapy. His professional career helps to understand the place of these practices in the psychiatric medicine field in Rio de Janeiro. The conclusions of this study suggest the parallel use of organicist theories and treatment methods based on therapeutic occupations in his professional practice. The trajectory of Heitor Péres helps to problematize the history of Brazilian psychiatry of the 1940s and 1950s, as well as to demystify the idea that praxitherapy was perceived in this context as a method of treatment inferior to those based on organicism. En este artículo se realiza un estudio historiográfico sobre el psiquiatra Heitor Péres, quien, entre otras funciones asumidas en el servicio público, dirigió la Colonia Juliano Moreira entre 1946 y 1956. El estudio tuvo como principal interés investigar las contribuciones de este personaje a la praxiterapia, el uso de ocupaciones terapéuticas dentro de la atención psiquiátrica. Heitor Péres fue un gran promotor y difusor de lo que llamó praxiterapia integral, además de la terapia de arte. La trayectoria de Heitor Péres ayuda a problematizar la lectura histórica que se ha hecho de la psiquiatría brasileña de los años 1940 y 1950, así como a desmitificar la idea de que a la praxiterapia se la percibía en este contexto como un método de tratamiento inferior a los basados en el organicismo. La historia demuestra que crisis graves como esta pueden resultar determinantes para dar origen a transformaciones importantes en la sociedad. El objetivo principal de este artículo es aportar reflexiones sobre las posibles consecuencias de la pandemia por COVID-19 en el futuro de la terapia ocupacional a partir del análisis de otras situaciones de emergencia ocurridas en la primera mitad del siglo XX


The COVID-19 pandemic has triggered an unprecedented global emergency. History shows that serious crises such as this can be instrumental in bringing about major transformations in society. The main objective of this article is to provide reflections on the potential impact of the VOC-19 pandemic on the future of occupational therapy based on the analysis of other emergencies that occurred in the first half of the 20th century

Background: Occupation-based models are generic explanations of occupational engagement. Their associated diagrams are conceptual tools that represent the key concepts and their interrelationships, which have withstood substantial shifts in the profession's knowledge base and scope over the last 30-40 years.Aims: We aim to bring into question the sustainability of the diagrams used to represent models.Methods: Intellectual history and semiotic analysis are combined as tools for examining the history of selected occupation-based models and the convention of representing them diagrammatically. Our critique employs a hermeneutically inspired semiotic technique to scrutinise the diagrams as stand-alone symbolic objects.Results: We argue that the rigid categorisation and oversimplified structure of diagrams keeps the profession pinned to dated perspectives based in positivism and dualism, bypassing the real, lived experiences of people. Our critique highlights the ontological absences from diagrammatic representations of occupation-based models.Conclusions: The continued practice of depicting models with diagrams needs to change, to create space to integrate other theory and perspectives, such as a more fundamentally human, ontological perspective.Significance: An ontological perspective is important for practice to advance past the dualistic or pluralistic stance the profession has held for decade, to understand how people experience their world rather than how the profession sees a person's world.

RESUMO

This article identifies and discusses five philosophical beliefs expressed in the writings of Eleanor Clarke Slagle, a pioneer and founder of the occupational therapy profession. Her beliefs express a positive view of people's ability and capacity to change their habits and routines to better enable them to function and participate in community and social life. Slagle believed that clients should have an evaluation and intervention program based on individual needs and goals whether the program was done individually or in groups. She believed that intervention programs could be organized into levels to form a progression from those who had deteriorated in daily habits to those ready for discharge back to the community. She believed that the education of the practitioners should include lectures for knowledge, application, and training in occupations to specific problems seen by practitioners, and practice training in the field under the supervision of trained occupational therapists. Her beliefs continue to underpin the practice of occupational therapy today.
